Service Description

This 50-60 minute session (via phone or video call) identifies all the health care needs and options of your elderly loved one. No two people are the same; therefore, no two circumstances have the same care plan goals. This consultation is a quick and valuable guide to prepare you for the next best step in managing the care of your elderly loved one. Prior to the conversation, you will be sent a questionnaire that is designed to better help us identify the needs of your aging loved one. It will be sent to you in a welcome email upon your checkout and should be completed 48hours prior to the initial phone session. Following the initial conversation, two 20-30 minute follow-up phone calls are scheduled to review the summarized care plan based on the goals that were identified. Recommendations for a solid plan of action to best accomplish your care plan goals will be listed in an easy-to-read format. You will also receive a helpful binder full of resources and tools to help you navigate the items outlined in the care plan proposed. This is all designed to empower you with the information you need to make the best care decisions for your aging loved one.
